It really is just the look from what I’ve gathered. Some go with smooth for ease of cleaning and a less “busy” look. I personally went with the pic, because I just liked the way it looked. I believe it’s all the same attachments no matter which way you go. Hope this helps.

They're both picatinny, kinda. The cheese grater one can use indexed picatinny mounts all across the rail, while the smooth can use them where you see those slots. A non-indexed picatinny mount will work all across either of them.
So... basically looks, yeah. One is slightly more versatile, but it doesn't really matter if you get non-indexed mounts.
