Make a Yahoo junk email to use for things like this, not one you actually use. I have like 5 or 6 email addresses, really only use 2 for legit communications.

Sure. Alternatively, vendors could just not ask for personal information when they want us to fill out a survey that only benefits them.

Looks like they modified the survey, it does ask at the end for name, email but it's optional. You're call if you want to do it of course, I didn't, get enough spam mail as it is.
