I have a shift knob, reverse lockout collar, and height extension in my subaru from lathewerks.com. They are all handmade on a manual lathe and are very high quality. I contacted them about having a knob made for the Bronco. They don't have it listed, but they do for the mustang and also do custom work.

You’re my hero!I am working on making my own custom leather shift boot. I've got the thing disassembled, next step is to cut it all apart and from there I can make a pattern from which to cut new leather. When I have the pattern I will send it to redline so that others can benefit from it.
