Are there any bike racks that:
1. Don't block the rear camera when they are folded up
2. Can allow for opening of the rear door, even if that means an additional swing adapter

Yea there are multiple swing aways that will work with the Kuat Pivot and 1Up has one as well I think. For the camera most likely would have to relocate to top of spare.

This is a Saris MHS 2 bike base with one tray and using the Saris Swing Away.
The swing away is mounted upside down. This lets it open to the driver side and not interfere with the tailgate.

Can I open the door? Yup.
Can I see out the rear with it folded up? ... Sorta.
It does block the very center of the screen, and makes the backup sensors beep. But it's wide enough you can still see a good amount, but not directly behind you.
Also, even if you turn off the backup sensor in the settings screen on the radio, it seems to not work after you turn the vehicle off and back on. The option still shows them disabled, but they do indeed still beep at you. You have to turn it off every time you go in to reverse the first time after start up.

It's pricey, but I was able to get them all with a 50% off coupon (that's since expired).

One of the big issues is that the hitch isn't that far down from the spare, so you'll probably need a drop receiver of some type.

Here's an option that I have contemplated. I am interested in 'swing arm' bumpers but the thought of opening a swing arm, a bike rack on a swing out, and the back door sounds like a hassle. This is the only integrated wheel/pakrax/maxtrax carrier I have seen with a second hitch dedicated to accessories not towing. And still leaves the main hitch exposed if you do want to tow. Looks interesting, not sure if it's the right fit.

The Hollywood SR1 is a spare tire mounted rack, so it gets out of the way when you open the tailgate. Currently using it with the Thule Tram for skiing and after testing it, it doesn't move around at all even on the highway.

They're out of stock on their site, but you can find them on Amazon.
